contracts with most major insurance organizations in the area. Patients
are asked to bring the insurance card to each visit so that changes
can be made to billing information on file to ensure prompt and accurate
filing. Patients are expected to pay co-payments at the time of service. The fee discount opportunity, based on Federal Poverty Guidelines, may be available to certain individuals with insurance coverage. Deductibles may be eligible for discounts after insurance is filed and payment information received. Discounts are calculated based on the total household income and family size using the same criteria as is used for other patients.
