The fee discount opportunity is based on current
Federal Poverty Guidelines. It is calculated based on the total
household income and family size. The following summarizes
certain requirements. Office Managers can answer specific questions
a patient or family might have.
What is a "family?"
A group of two or more persons related by birth, marriage, adoption
who live together. All such related persons are considered as members
of one family.
What is included as
"Income"?
Gross income - or the amount before any deductions - is used to
determine eligibility. Anything that is received by any family
member must be included to calculate total household income. For
a complete list, contact the Office Manager. Generally, however,
you must bring proof of wages and salary earned, benefits payments
such as SSI, unemployment, disability, retirement, etc. Examples
of acceptable documents are: W2s, check stubs, letter from
DFCS, Worker's Compensation award, etc.
When must I provide documentation?
You should bring documentation at your first visit, anytime your
situation changes, and at least the first visit of each calendar
year.
Does my discount change?
If your income, household size, or the Federal Poverty Guidelines
change, your discount will be recalculated.
Can I be considered
for discounts if I have insurance or Medicare?
After insurance is filed and paid, any amount owed by the patient
may be considered for discounts providing that proof of income and
household size has been given.
