For well-child visits beginning at birth up to 2 years of age, our providers will check length, weight, head circumference, temperature, and heart rate. Gathering this information will help determine where your child falls on the percentile chart for children of a similar age. Developmental milestones and social history will be reviewed at each well-child check. The provider will check out different parts of your child’s body to ensure there are no major issues. At 2 years of age, blood pressure will become part of this annual visit.

Well-child visits can start 2 – 3 days after being discharged from the hospital. Visits will continue at 2 weeks, 1 month, 2 months, 3 months, 4 months, 6 months, 9 months, 12 months, 15 months, 18 months, 24 months, and 30 months. After 30 months, there will be a 3-year check that will transition into a well-child check on an annual basis.

For parents or guardians, requests for immunizations records can be done by phone, office visit, or through the patient portal. Please allow 48 – 72 hours for turnaround.
